What is neighbourhood working?

Neighbourhood working brings communities, health, social care and voluntary organisations together to improve the health and wellbeing of a local community.

This is a new way of working and the ‘joined up approach’ is already making a positive difference for local people.

Each community will be invited to create their own vision of what a healthy neighbourhood looks like. We want you to live a full and active life with easy access to the support, care and treatment you may need.

Health and social care providers and voluntary organisations will work more closely together in Local Neighbourhood Teams (LNTs) to help communities bring their vision to life.

Using local knowledge and local health data, an action plan will be established using the existing strengths of the community as building blocks.

Neighbourhood working will tackle some of the social and economic problems that can affect our health.  It will help to reduce inequalities and create more patient-focussed health and social care services for each community.

Our environment: 5%

Healthcare: 10%

Social and economic circumstances: 15%

What is a Live Well Neighbourhood Team?

The core Live Well Neighbourhood Team (LNT) is composed of a manager, community connector and engagement lead with representatives from key local organisations. These members come together regularly to discuss the cases of those citizens that would benefit from the support of more than one organisation and see how they can most effectively work together to provide that support.

Other organisations may participate in these meetings depending on the needs of the individual or family concerned.

A Live Well Neighbourhood Team has already been set up in Colchester Central and other areas will follow. The Colchester Central Neighbourhood includes the town centre, New Town, Berechurch, Prettygate, Shrub End and Stanway.
