Age Well East is a charitable organisation focussed on improving quality of life for individuals in later years. Since April 2020 our services have expanded to work in partnership with Alliance colleagues to offer volunteer led community support services for people with mental wellbeing concerns around end of life and bereavement.

East of England Ambulance Service (EEAST)

The East of England Ambulance Service NHS Trust (EEAST) has a vital role in delivering high quality care to patients at the end of their life.  As a Trust, we need to ensure that patients not only receive a timely response but also the appropriate emergency care or transport to hospital. Patient choice, including decisions around resuscitation and place of death should be supported by the ambulance service staff.

Healthwatch Essex is an independent voice for the people of Essex. Healthwatch Essex gathers and represent views about all aspects of health and care in the county to help shape services.

Healthwatch Essex can help you find out about and access health, social care and wellbeing services through its information service, including all matters relating to end of life care support for the individual, carers and loved ones. This can include details on accessing such services as support groups, hospices, counselling and care packages. Friendly and knowledgeable information & guidance officers take the time to listen and provide options tailored to the individual. How to get in touch:

St Helena

St Helena helps local people face incurable illness and bereavement by reaching out to members of our community, helping them make their own choices and live with dignity. The aim to bring comfort and relief to all those who need it, offering the people of north essex individual care and total support, regardless of their diagnosis or personal circumstances.

St Helena provides its expert and compassionate care at The Hospice in Colchester, over the phone, and across north essex in the patient’s home; with the aim of supporting as many people as possible in their preferred place of care.

For those who wish to remain in their usual place of residence, physical, emotional and spiritual support is provided from the Hospice in the Home teams including through the SinglePoint 24/7 advice line and Virtual Ward service, providing personal care in the last weeks of life.

St Helena oversees the My Care Choices Register, where individuals’ decisions about their choices for care, including end of life care, and their preferred place of care is recorded. St Helena works with other local health and social care partners to promote the My Care Choices Record to ensure those who need to be are on the register.

St Helena’s priorities:

  • Reaching out based on need regardless of diagnosis or circumstances
  • Empowering people to plan ahead, share their choices and achieve their wishes
  • Providing excellent personalised care to more people in hospice beds and in the home
  • Helping life go on in the face of dying, death and bereavement
