My Care Choices Register

The My Care Choices Register is for people in north Essex who are living with incurable illness, dementia or frailty and who wish to record their priorities for future care.

The My Care Choices Register is an electronic record of a person’s decisions about the kind of care they wish to receive in the future if they were more unwell and their preference for the place of care at end of life.

People can record their wishes by filling out a My Care Choices Record.

Once a record is filled out, consent can be given for the patient’s GP record their choices on the Register. This information can be seen by GPs, out of hours GPs, community nurses, the hospital, St Helena Hospice, the ambulance service and some care home staff.

Having the conversation and planning for end of life care has shown to be transformative for those involved.  It can be an upsetting conversation to have but people are glad to have had it and feedback has shown that for those with an end of life care plan, the care they receive is better.
