St Helena
St Helena helps local people face incurable illness and bereavement. St Helena SinglePoint also acts as a hub for referrals for end of life care. Referrals can be made via St Helena’s website for the following:
Via the hospice services professional referral form:
- Referral to community end of life team for pain/symptom control, emotional, psychosocial, spiritual, other
- Referral for care in home for last 6 weeks of life
- Request for inpatient hospice bed (ESNEFT patients via palliative care team only)
- Request for district nurse/community ANP (matron) visit e.g. catheter care, wound dressings for all patients in the last year of life
Via the breathlessness referral form:
- Referral for specific breathlessness clinics or groups
Via the ‘refer someone else for bereavement support’ form:
- Referral on behalf of another adult for bereavement support
Age Well East
Age Well East aims to boost the welfare and quality of life of Colchester borough’s older people – and those who care for them, through advice, information, knowledge and where appropriate, resources.
Referrals for their end of life care services including compassionate companions; bereavement support; and dementia support can be made via their online referral form.
East Suffolk and North Essex Foundation Trust
ESNEFT gives support to patients who have a life-limiting or life-threatening condition and also to their families.
Services include:
- management of pain
- management of other symptoms
- psychological support
- social support
- spiritual support
